These Terms of Service govern your use of the Forward Benefits website, operated by Forward Benefits, a division of Bridge Forward / Ready App, Inc. By using this website or submitting a Benefits Check, you agree to these Terms.
Not an insurer; advertising only
Forward Benefits is not an insurance company, insurance agency, or licensed insurance producer. This website contains general information and is an advertisement, not a contract or an offer of insurance. Coverage is underwritten by Aflac and administered by PIOPAC Fidelity Insurance, a licensed Third-Party Administrator, and enrollment is conducted exclusively by licensed insurance professionals from The Trusted Consultants. Nothing on this site is a recommendation of a specific insurance product or an offer to sell insurance.
The Benefits Check
Submitting a Benefits Check is a request for a licensed advisor to contact you. It does not create any coverage, enrollment, or obligation to purchase. Not all products are available in all states, and benefits, premiums, and eligibility vary by state. Any coverage is subject to the applicable policy terms, conditions, limitations, and exclusions, which your advisor and the carrier will provide.
Communications consent
If you opt in, you agree that The Trusted Consultants may contact you by phone and text, including with automated technology, at the number you provide. Consent is not a condition of purchase. Message and data rates may apply and message frequency varies. You may opt out at any time by replying STOP or calling (866) 915-5499.
Accuracy of information
You agree to provide accurate, current information and to submit only your own contact information or information you are authorized to provide. We may decline to process submissions that appear inaccurate, automated, or fraudulent.
Intellectual property
The content, design, and marks on this website are owned by Bridge Forward / Ready App, Inc. or used with permission and may not be copied or used without authorization. Third-party names, including Aflac, PIOPAC Fidelity, BenefitHub, and The Trusted Consultants, are the property of their respective owners and are used for identification only.
Disclaimers & limitation of liability
This website is provided “as is” without warranties of any kind. To the fullest extent permitted by law, Bridge Forward / Ready App, Inc. is not liable for any indirect, incidental, or consequential damages arising from your use of this website. Insurance coverage decisions and payments are the responsibility of the applicable licensed carrier and advisor, not of Forward Benefits.
Changes to these Terms
We may update these Terms from time to time. Continued use of the website after changes take effect constitutes acceptance of the updated Terms.

Accidental Death and Dismemberment (AD&D) insurance pays benefits only for death or covered losses resulting from a covered accident. It does not cover death due to illness, disease, or natural causes. AD&D is a supplemental product and is not a substitute for life insurance. Term life products are subject to underwriting and eligibility requirements.
The supplemental insurance products described here provide limited benefits for specific covered events such as accidents, critical illness, or accidental death. These products are not comprehensive health insurance and do not cover most routine health care costs.
